Once received, Village Counselling will make contact with your patient.

Alternatively, please give the completed Mental Health Care Plan to your patient and advise them to call Village Counselling to secure an appointment.

Please indicate on the referral the number of sessions authorised – 6 or 10.

A treatment report will be forwarded to you following the completion of the number of sessions authorised.
